1. GENERAL PATIENT INFORMATION

Passport details. Age is of great significance for pregnancy, delivery and perinatal outcomes prognosis. Thus, young (under 18) primaras have immature skeleton, narrow pelvis and delivery often ends up with cesarean section. They have a higher risk of preeclampsia, miscarriage and premature delivery. Natural delivery can be accompanied by such complications as premature rupture of membranes, abnormalities of labor, bleeding in the third and postnatal period, fetal hypoxia. Women after 35 have a significantly higher risk of delivering a child with chromosomal abnormalities (Down's syndrome, Edwards' syndrome, Patau's syndrome, etc.) and with monogenic hereditary diseases. One should also mention somatic burden (chronic arterial hypertension, diabetes mellitus, impaired fat metabolism, etc.) in women of older reproductive age (35-45 years), which significantly increases perinatal and maternal risks. Apart from that, pregnancy in such patients is often accompanied by clinical signs of placental insufficiency, intrauterine growth restriction factor. Complications in labor and postnatal period are seen much more often.

Admission channel (delivered by the ambulance team, referred by a prenatal clinic doctor, admitted without medical order).

Date and time of admission to an in-patient facility. Time of admission is mandatory as it is crucial to observe strict timing with thorough control of stage-by-stage approach to delivery of care and necessary clinical and laboratory examination and management according to the criteria of medical care quality assessment and adopted algorithms of management of women with extragenital abnormality and complications in pregnancy.

Diagnosis at admission. The history of pregnancy and childbirth shall contain the diagnosis of the referring prevention and treatment facility [prenatal clinic, obstetric or other in-patient facility (if a woman is transferred)] or of the ambulance team. In cases of hospitalization without medical order the diagnosis of the maternity hospital reception ward.
